Online Membership Form

New members should provide all contact and Membership Directory information. Dues will not be collected with this form! Dues may be paid by going to phsne.org/paypal or mailing a check or money order in US funds to PHSNE at address on form.
Renewing members should make sure all contact and Membership Directory information is up to date. Dues may be paid by going to phsne.org/paypal or mailing a check or money order in US funds to PHSNE at address on form.
Active members, not paying dues at this time, may use this form to update any contact and Membership Directory information that may have changed.

To complete this membership form online, please scroll through the form window below and fill out all required fields, indicated with an asterisk (*), then click the Submit button. If successful, you will receive a "Thank You" confirmation. If you missed a required field, the form will return with an error message. Please scroll down to find and fill in any empty required fields, which will be highlighted, then click Submit again.

Note that the form window has it's own scroll bar to move down the form within it's frame. If your browser does not display the form scroll bar, or you prefer to work with the form in a new secure window, click on new form window. If you open the form in a new window, you will be submiting your information over a secure (https) connection, which you can verify by the lock symbol most browsers display for a secure page and the presence of "https://" at the beginning of the URL shown by your browser.
